Covid-19 Information

We have worked hard to implement a protocol at our office to keep you and our team safe. Please be aware of the following changes:

 

• As of October 14, the governor has lifted the mask mandate in healthcare facilities, unless a community has a high transmission rate. Therefore, masks for patients and staff are optional. We will be continually monitoring the risk level of our area and adjusting this policy accordingly. If you feel more comfortable having the staff wear a mask around you, please ask!

• In order to maintain proper social distancing, we have reduced the number of patients we see each day. Because this greatly limits our availability, we are implementing a $50 fee for no showing or late cancellation/rescheduling (excluding emergencies). We appreciate your cooperation in giving at least 24 hours notice if you are unable to make your appointment.​

• If you are coming in to pick up your eyewear, please call the office first to schedule an appointment. ​

• All check in forms will be available on our website
